Our Long Term Care Program is aimed at sustaining each residents maximum physical, social and cognitive function. The staff at Hopkins are frequently in-serviced by our in house education coordinator to ensure the highest level of care is given. Our professional nursing staff are available 24 hours a day Seven days a week as well as our in house physician and nurse practitioner.
Weekly meetings are held to ensure that all patients have up to date care plans dealing with each persons individual well being. This is completed with the staff as well as the residents family to guarantee our residents are reaching their highest potential. Care plans are designed to incorporate all aspects of care for a person such as social service, recreation therapy, rehabilitation, dietary and nursing amongst others.

All visits will require 24-hour advance registration.
❏ Our suggested visitation hours at this time are: Monday – Thursday 10:30-6:30, Friday
10:30-4:30, Saturday and Sunday 9:30-4:30
❏ Visits are limited to 30 minutes per guest, during peak hours.
❏ Visitation spaces will be made available indoors (All spaces are handicap accessible).
❏ Unfortunately we cannot accommodate our guests with the use of our restrooms.
❏ We request visitors to wear a mask throughout the visit; extra masks available on-site.
❏ All guests are requested to maintain a distance of 6-feet from other visitors.
❏ If any visitor fails to adhere to the protocol, he/she/they will be prohibited from visiting for
the duration of the COVID-19 state declared public health emergency
❏ We request all visitors share their address, phone number, and email address upon
scheduling your appointment so we can stay in touch!
