Notification to Residents, Representatives, and Family Members

To Our Residents and Family Members:

In compliance with Governor Andrew Cuomo directives and  Department of Health’s guidance, Hopkins Center for Rehabilitation and Healthcare, will post on a weekly basis all residents to date, who have infections and the number of deaths.

Week of January 8, 2021 Hopkins Center for Rehabilitation and Healthcare has:

 7 residents with infections

2 resident deaths.

We know that you may have questions and we encourage you to contact our center. Please call us at 718- 694 -6700, or check our website at www.hopkinscenter.com  for daily and or weekly updates.

We are excited to announce we are OPEN for In-Person visits accommodating all of our guests!

Please self-schedule your visit by clicking the button below.  If you need assistance with making a visitation appointment, please call our Visitation Facilitator,
Sabena Belle, at 929-399-9692.

Visitation space is available outdoors and indoors.
❏ We request visitors to wear a mask throughout the visit; extra masks available on-site.
❏ All guests are requested to maintain a distance of 6-feet from other visitors.
❏ If any visitor fails to adhere to the protocol, he/she/they will be prohibited from visiting for
the duration of the COVID-19 state declared public health emergency
❏ We request all visitors share their address, phone number, and email address upon
scheduling your appointment so we can stay in touch!
Visitation appointments are available 7 days a week.
Please contact our Visitation Facilitator, Sabena Belle at 929-399-9692, to pre-schedule your visit appointment.
Walk-ins are welcome, however we suggest you call our Visitation Coordinator prior to your visit.
Compassionate visits/In-room visitation will be granted on a case by case basis.
